What It Is

TB-500 is a synthetic fragment of thymosin beta-4, a repair protein the body already makes. Until now it was only on our formulary inside the combination vials (Wolverine Stack, BPC-157/TB-500/KPV, KLOW). This is the standalone vial, added when Optimal Balance Pharmacy listed it separately in July 2026.

How It Works

Thymosin beta-4 regulates the actin cytoskeleton, which is what lets repair cells migrate to damaged tissue. It also promotes angiogenesis and dampens inflammatory signaling. The action is systemic rather than local, which is why it complements BPC-157 instead of duplicating it.

Who It's For

Patients with widespread or recurring soft-tissue injuries, patients already running BPC-157 who want to add the systemic half, and cases where the provider wants to dose the two peptides independently rather than at the fixed combination-vial ratio.

Who Should Avoid It

Flag active malignancy for provider review; TB-500 is angiogenic. Monitor autoimmune conditions. Prohibited in tested sport at all times under WADA category S2.

Protocol & Pricing

OBP Pharmacy Price

$80/50mg

You pay pharmacy price. No markup.

Starting Dose

20 units (2mg)

Form

Injectable vial (5mL, 10mg/mL, 50mg total)

Dosing Protocol

20 units subcutaneous injection twice weekly during the loading phase. 20 units = 0.2mL × 10mg/mL = 2mg per injection. Loading runs 4 to 6 weeks; maintenance then steps down to 2mg every other week per your provider. The 50mg vial covers a full loading course.

Beyond Use Date

90 Days from compounding (USP <797>)

Optimal Balance Pharmacy assigns this date and prints it on your vial label. Always follow the label. Once a multi-dose vial is punctured, the in-use date on the label is shorter than the compounding date shown here.

How to get TB-500 prescribed online

TB-500 is available by prescription through a licensed telehealth visit and dispensed for human use by a named 503A pharmacy. That is what separates it from research-use-only vials sold without a prescription. There is no subscription and no membership, just a one-time visit fee.

  1. 1

Complete a $39 medical visit. Answer a short intake about your health history and your goals for TB-500. The $39 fee is the only charge RxPepsDirect collects, paid once, not a recurring subscription.

  2. 2

A licensed provider reviews it. A US-licensed nurse practitioner or physician reviews your intake and, if TB-500 is appropriate for you, writes a patient-specific prescription. The starting dose is typically 20 units (2mg).

  3. 3

The pharmacy compounds and ships it. Optimal Balance Pharmacy, a 503A pharmacy in Texas, compounds TB-500 and ships it pre-reconstituted by FedEx overnight in a reusable cooled case. Your fill is Injectable vial (5mL, 10mg/mL, 50mg total), and you pay the pharmacy $80/50mg at wholesale, with no clinic markup, plus a $15 flat FedEx overnight shipping fee charged by the pharmacy.

Common questions about TB-500

What is TB-500?
TB-500 is a synthetic fragment of thymosin beta-4, a repair protein the body produces naturally. It binds actin and regulates the cytoskeleton, which is the step that lets repair cells migrate into damaged tissue. It also supports new blood-vessel formation and reduces inflammatory signaling. Optimal Balance Pharmacy compounds it as a 50 mg vial at 10 mg/mL, shipped pre-reconstituted and ready to inject.
How is standalone TB-500 different from the Wolverine Stack?
Same molecule, different packaging. The Wolverine Stack is BPC-157 and TB-500 combined in one vial at a fixed 3 mg/3 mg per mL ratio, so every dose moves both peptides together. The standalone vial lets your provider set the TB-500 dose independently, which matters if you are already running BPC-157 separately or if your provider wants a different balance between local and systemic repair. Do not run the standalone vial at the same time as any combination vial that already contains TB-500.
How do I dose TB-500?
The common pattern is a loading phase of 2 mg twice weekly for 4 to 6 weeks, then a maintenance dose of 2 mg every other week. At the 10 mg/mL concentration, 2 mg is 20 units (0.2 mL) on a U-100 insulin syringe. The 50 mg vial covers a full loading course. Your prescriber sets your actual dose and duration.
Can I combine TB-500 with BPC-157?
Yes, and that is the most common pairing. BPC-157 drives local tissue repair and angiogenesis; TB-500 promotes systemic cell migration. They work on complementary pathways, which is why they are sold together as the Wolverine Stack. Running them as two separate vials gives your provider independent control of each dose. What you should not do is run standalone TB-500 alongside a combination vial that already contains it.
Is TB-500 banned in sport?
Yes. TB-500 is prohibited at all times under WADA category S2. If you compete in a tested sport, tell your provider before starting. This applies to the standalone vial and to every combination vial that contains TB-500.
Who should not take TB-500?
Active malignancy is flagged for provider review, because TB-500 promotes blood-vessel formation. Autoimmune conditions are monitored rather than automatically excluded. Human trial data is limited and most supporting evidence is preclinical, so your provider weighs that in the decision. RxPepsDirect provides the telehealth prescription; Optimal Balance Pharmacy fills it and ships directly to you.
