Why intranasal delivery exists

The blood-brain barrier is very good at keeping large molecules out of the brain, which is a problem when the molecule is a peptide and the target is inside the brain. The upper nasal cavity is an unusual piece of anatomy: the olfactory and trigeminal nerves terminate there and run directly into the central nervous system, providing a route that partly avoids that barrier. That is the entire pharmacological argument for a nasal peptide, and it is a strong one when the target is central. For a peptide acting on a joint, a tendon, or fat tissue, the nasal route offers nothing an injection does not do better.

Nasal spray formulations carry the shortest dating we dispense: 30 days from compounding. That is a real constraint rather than a technicality, and it is worth planning around. A patient who orders a nasal spray and then waits two weeks to begin has already lost half the usable life of the bottle. A patient using a spray intermittently, which is common for PT-141, will very likely discard product. Where a compound is offered in more than one format, as oxytocin is in both a nasal spray and a 180-day troche, the dating difference should factor into the choice as much as the route does. Is PT-141 better as a nasal spray or an injection?
Both are prescribed and the choice is largely about tolerance and convenience rather than efficacy. The nasal spray ($99 per 15mL) avoids needles and is often preferred for an as-needed medication. The injection gives more precise dosing, which matters because PT-141 side effects including nausea and blood pressure changes are dose-related. Patients sensitive to nausea sometimes do better on the injectable simply because the delivered dose is more predictable.
How do I use a peptide nasal spray correctly?
Technique affects how much dose you actually absorb, so it is worth getting right. Aim the tip toward the outer wall of the nostril rather than up toward the septum, breathe in gently rather than sniffing hard, and do not tip your head back. Sniffing forcefully drives the spray down the throat, where it is swallowed and largely wasted. Your prescriber will confirm the specific dose and frequency for your prescription.
Are peptide nasal sprays safe?
The delivery route adds its own effects on top of whatever the peptide does: local irritation, dryness, and occasionally a bitter taste when some of the dose reaches the back of the throat. Those are usually mild. The safety profile of the peptide itself is unchanged by the route, so PT-141's blood pressure and nausea effects apply whether it is sprayed or injected.
